Nominees may be PGY1, 2 or 3 level, and can be nominated by a colleague, or chose to self-nominate. Application forms should be returned to PMCQ via email, to jmof@pmcq.com.au or fax, to: (07) 3131 6850 by no later than 30 June 2010. Please note there is no limit to the number of applicants per Facility/District.
